Accident Reporting

Accident Report Form


Please fill in a hard copy of the form below. The hard copies can be found in the Club Foyer, the Training Room and the Boatshed.

Once the form has been completed, please put it in the incident form box in the Club Foyer and inform office@wksc.org.uk . In the subject line please put ACCIDENT REPORT including the time and date. The appropriate Senior Club Officers and Risk Control Coordinator will then be informed.

Filling in the form:

- The form is self explanatory. If in doubt ask for help. 

- Clearly and accurately record the facts of the incident. Do not record theories or speculation on the causes of the accident, particularly to not apportion blame.

- Record the names of any person(s) who was/were injured and any direct witnesses to the incident.

- If possible, sketch the area where the incident occurred, and/or take photographs where possible if it is safe to do so.
